FERPA Release

Used to authorize release of students’ advising records and related academic information to third parties

Designated Pronoun Opt-Out

Used if a student does not want their designated pronoun shown in the LSA Student Advising File and related applications

Petitioning the Academic Standards Board

Used to request exceptions to college policies

Drop/Add

  • Petition for an Exception to the Late Add Deadline:
    Used during Period III Drop/Add.
  • Request for Term Withdrawal:
    Used to withdraw from the term once classes have begun.
  • Petition for Retroactive Course Drop:
    Used for requested course drops after classes end for that term. Approval is extremely rare and is only granted for extraordinary circumstances. There is a one-year deadline and a case must be made that the student was unable to complete the class and unable to request the drop during the term due to a non-academic event or issue. An explanation is necessary for the request for a specific class, but not all classes.
  • Petition for Retroactive Term Withdrawal:
    Used to withdraw from the term after the last day of classes. There is a one-year deadline and a case must be made that the student was unable to request a withdrawal during the term due to a non-academic event or issue.

Requirement Substitutions and Exceptions

  • Bachelor of Science Eligibility Petition:
    Used to request that a course be counted as Bachelor of Science credit.
  • Distribution Requirement Petition:
    Used to request a distribution requirement substitution.
  • Exception to Winter 2020 Semester Grade Uncovering Deadline.
  • Race & Ethnicity Requirement Substitution Petition:
    Students must meet with a member of the Race & Ethnicity committee before submitting a petition.
  • Quantitative Reasoning Requirement Substitution Petition:
    Used to request a QR requirement substitution.
  • Writing Requirements:
    LSA’s writing requirements are administered by the Sweetland Center for Writing. Consult their website for information about special arrangements for the writing requirements.

Incompletes and Modifications

  • Request for Modification of Credit Hours:
    Used if a student is modifying a course within the credit-hour range that is allowed for the course or if they want to take a course for more credit hours than the course is allowed.
  • Petition for Late Modification of Credit Hours After Late Drop Deadline:
    Used after the late drop/add deadline to modify course credits. Find current term deadlines on the Academic Calendar. 
  • Request for Audit Status:
    Used if a student is seeking to officially audit a course.
  • Application for Retroactive Credits:
    Used to apply for language retroactive credits if a student has met the guidelines. 
  • Time Extension for Incomplete:
    Used if a student has an Incomplete and extenuating circumstances causing them to need an extension beyond the deadline.
  • Instructor-Student Agreement for Finishing an Incomplete Course:
    Used if a student or instructor are making arrangements for an incomplete.

Readmission/Reactivation

Used to apply for reactivation if the student has been away from the College for more than 2 calendar years (24 months).

Undergraduate Multiple Dependant Degree Program (MDDP)

Used to apply for a Multiple Dependant Degree Program with LSA and another school or college.

Dean's Recommendation Form

Used to fulfill requests from professional and graduate schools, professional associations, and prospective employers who may require Dean’s Recommendation forms. The forms usually say something similar to the following: “To be filled out by College Dean or official”. Processing time for a Dean’s Recommendation is 2-3 weeks, please plan accordingly. 
The most common uses for the Dean’s Recommendation form are: 

  • Application to graduate and/or professional school
  • Application for non-University of Michigan study abroad programs
  • Application for transfer out of the University of Michigan to another institution

AP Credit Removal

Used to request the removal of AP credits from a student’s transcript if they decide they no longer wish to have the AP credits counted toward completion of their LSA degree. Credits will be removed only after consultation with an academic advisor.
