Used to request exceptions to college policies. Many of the forms below are able to be completed fully online and will link to the location allowing students to complete them.
If you need to submit a form that does not have an online process, please fill it out and email it to firstname.lastname@example.org. The Newnan Advising Center Front Desk will not accept paper copies of these forms
Used to apply for a Multiple Dependant Degree Program with LSA and another school or college.
Used to fulfill requests from professional and graduate schools, professional associations, and prospective employers who may require Dean’s Recommendation forms. The forms usually say something similar to the following: “To be filled out by College Dean or official”. Processing time for a Dean’s Recommendation is 2-3 weeks, please plan accordingly.
The most common uses for the Dean’s Recommendation form are:
Used to request the removal of AP credits from a student’s transcript if they decide they no longer wish to have the AP credits counted toward completion of their LSA degree. Credits will be removed only after consultation with an academic advisor.
Used to apply for reactivation if the student has been away from the College for more than 2 calendar years (24 months).
